Where to apply for a Passport

The DFA office located at 2330 Roxas Blvd., Pasay City. From Buendia LRT station take a jeep with a Holiday-DFA-Libertad sign. Alight at the corner of Libertad and Harrison then walk westward about 50 meters to the DFA office. Sa labas pa lang marami na fixers try to avoid them and go directly to the DFA .

Requirements

Personal appearance
Completed Application form

Birth Certificate in security paper from the NSO.
Atleast two government issued documents proving your identity and citizenship. Examples are drivers license, PRC ID, seaman's book, SSS id, income tax return.
